Client Consent / Waiver

Purpose of Life Coaching Individual Sessions:

I  understand that I am seeking Life Coaching individual sessions from Growth Life Coaching, for the purpose of personal growth, skill development, mindset transformation, habit improvement, and the achievement of individual goals.

Description of Services:

The Coaching Services may include but are not limited to:

• Identifying in overcoming limiting beliefs

• Enhancing personal skills

• Setting and achieving personal goals

• Managing control over personal habit  

 

Confidentiality:

I understand that all information shared within Coaching sessions will be kept confidential, except in the following circumstances:

• If I pose a risk of harm to myself or others

• If there is suspicion of abuse

• If required by law

Informed Consent:

I acknowledge that I have been provided with information about the nature and purpose of the coaching programs and sessions, including potential benefits and risks. I understand that Life Coaching is not a substitute for medical or psychological treatment, and I am encouraged to seek appropriate professional help if needed.

Voluntary Participation:

I confirm that my participation in Coaching sessions is voluntary, and I have the right to withdraw consent at any time without prejudice, in adhering to terms of any contract. I understand that I am free to ask questions and express concerns during the sessions. I acknowledge and agree that my coach has the professional discretion to challenge my thoughts, perspectives, and behaviors during our sessions. I understand that this process is intended solely to support my personal growth, development, and progress.

Release of Liability:

I release Growth Life Coaching from any liability arising from my participation in Coaching sessions. I understand that such techniques are not guaranteed to produce specific results, and that Individual results may vary depending on the client’s level of commitment, personal application of strategies, and external factors, including interpersonal or organizational circumstances beyond the coach’s control. The Life Coaching Practitioner reserves the right to refer a case to an appropriate professional if applicable.

Cancellation & Rescheduling Policy

At Growth Life Coaching, we value your time and commitment to the coaching process. Please review the following cancellation and rescheduling terms:

1. Rescheduling Allowance

   • Clients may reschedule one session per month without penalty, provided at least 36 hours’ notice is given via email/text/call.

   • Any additional reschedules within the same month will be treated as cancellations and subject to the cancellation fees below.

2. Cancellation Fees

   • 36 hours’ notice: No charge.

   • 36–48 hours’ notice: A fee equal to 25% of the session rate will be charged.

   • Less than 36 hours’ notice or no-show: A fee equal to 50% (less than 36hrs) - and100% (no show) of the session or package (*below) rate will be charged.

3. Prepaid Packages*

   • For clients on prepaid coaching session packages, cancelled sessions will be deducted from the package balance in accordance with the above terms.

By scheduling a session/package, you agree to these policies. This ensures fairness and respect for both your time and ours, while supporting your commitment to the coaching process.
